Here are the answers to questions 3-5:
3. The 21 windows represent a 21-gun salute in honor of those who lost their lives on December 7, 1941.
4. The flagpole is attached to what is left of the main mast of the USS Arizona.
5. The Arizona Memorial was designed by noted Austrian-American architect Alfred Preis. He also designed the ILWU (union) headquarters building, which we saw in "A Lion in the Streets" (Season 12). The building-wide mural was painted by Mexican muralist Pablo O'Higgins. See pictures from the mural.

I can answer number 2. The episode "Time and Memories" had scenes at the memorial. There were others as well. Such a solemn place.
